Thank you for considering The Amazing Pizza Machine for your request. Because of the great amount of requests we receive, please submit your request 3-6 weeks prior to your event and complete all of the fields in the form below.

How it Works

  1. Submit your request
  2. Our Donations Committee meets twice a month to review all requests
  3. If tentatively approved, you will receive an email or phone call to further discuss and finalize your request.
